BeautiFill vs. CoolSculpting: What’s the Difference, and Which One Is Right for You?

If you’ve been researching body contouring in Columbus, Ohio, you’ve probably come across both BeautiFill and CoolSculpting. They’re often mentioned in the same breath, but they work very differently — and for the right patient, the difference matters a lot.

As the physician performing BeautiFill procedures at Retro Health & Aesthetics, I want to give you an honest comparison. Not a sales pitch — just the clinical picture, so you can make a decision that actually fits your goals.

What Is CoolSculpting?

CoolSculpting (cryolipolysis) uses controlled cooling to freeze and destroy fat cells. The treated fat cells die over time and are gradually cleared by your body over the following weeks to months. It’s non-invasive — no needles, no incisions — and has been widely available in med spas and dermatology offices for over a decade.

It works best on smaller, pinchable pockets of fat. Results take 2–4 months to fully appear, and multiple sessions are often needed to reach the desired outcome.

What Is BeautiFill?

BeautiFill is an FDA-cleared laser-assisted liposuction system by Alma Lasers. Using a 1470nm laser fiber, it liquefies and removes fat cells in a single in-office procedure — performed awake, under local anesthesia only, with no hospital or surgery center required.

What makes BeautiFill genuinely different is what happens to the fat after it’s removed. Unlike traditional liposuction, BeautiFill preserves up to 95% of fat cell viability during extraction. That means the harvested fat can be immediately re-injected into areas where volume restoration is desired — the face, hands, or as a natural alternative to a Brazilian butt lift.

Who Is a Better Candidate for BeautiFill?

BeautiFill tends to be the stronger choice when:

  • You have a more significant volume of fat to address — not just a small pinch
  • You want results from a single treatment rather than multiple sessions
  • Skin laxity is a concern — the laser component helps tighten as it treats
  • You’re interested in fat transfer to another area (face, hands, or natural BBL)
  • You want the procedure performed by a physician who knows your full health history

Who Might Be a Better Candidate for CoolSculpting?

CoolSculpting may be a reasonable option when:

  • The target area is very small and well-defined
  • You prefer a completely non-invasive approach with zero recovery time
  • You’re not interested in fat transfer
  • You’re comfortable with a gradual process over several months and multiple visits

A Note on Paradoxical Adipose Hyperplasia

This is something I think patients deserve to know about. CoolSculpting carries a rare but real risk called paradoxical adipose hyperplasia (PAH) — a condition where the treated area actually enlarges rather than shrinks, resulting in a firm, enlarged mass of fat. It occurs in roughly 1 in 138 patients based on some studies, and it requires surgical intervention to correct. BeautiFill does not carry this risk, since the fat is physically removed during the procedure.

What We Offer at Retro Health & Aesthetics

At Retro Health, BeautiFill procedures are performed by me — Dr. Bethany Recker, MD — in our private Worthington practice. As your concierge physician, I already know your health history, your medications, and your goals. That context matters when you’re making a decision about a body contouring procedure.

We don’t offer CoolSculpting, and that’s intentional. After reviewing the evidence and the risk profile, BeautiFill delivers more predictable, more permanent results for the patients I see — with the added option of fat transfer that no freezing technology can replicate.

If you’re weighing your options and want a physician’s perspective on what’s right for your body specifically, I’d love to talk. Consultations are $150 and credited toward any procedure.
